This article provides full details of the physical screening schedule and requirements for candidates who have applied for admission into the Federal University of Health Science Azare (FUHSA) for the 2025/2026 academic session.

Physical Screening Schedule

The physical screening exercise will be conducted at the university on the following dates:

Tuesday, 26th August, 2025	MBBS, BDS, Human Physiology, Human Anatomy and Biochemistry
Wednesday, 27th August, 2025	Nursing, Biotechnology, Microbiology, Pharmacology and Epidemiology & Biostatistics
Thursday, 28th August, 2025	BMLS, Physiotherapy, Optometry, Occupational Therapy, Speech Therapy and Human Nutrition
Friday, 29th August, 2025	Radiography, Audiology, Environmental Health, Public Health, HIM and Information Technology & Health Informatics

Required Documents

Candidates must bring the originals of all their documents and one set of photocopies. The required documents are:

▪️ Acknowledgement Form (printed from the FUHSA Post-UTME Portal)

▪️ UTME Result slip

▪️ ‘O’ Level Result (WAEC/NECO)

▪️ ‘A’ Level Result/ First Degree certificate (for Direct Entry only)

▪️ Scratch cards for ‘O’ level result(s) submitted

▪️ Evidence of Local Government Area of Origin

▪️ Birth Certificate or Sworn Affidavit of Age Declaration
